Healthcare for the Homeless Partnership
Ensuring homeless individuals and families receive appropriate wholistic health services

Established through a partnership between EUMA and the Faith Community Nurses of NW PA, HC4HP is a program centered on collaboration with community providers to offer integrated healthcare and social services to individuals who are homeless or impacted by poverty. This outreach program reaches clients that may not receive services otherwise by going directly where clients are, including: on the streets, at drop-in centers, shelters and soup kitchens.

·   Client centered, trauma informed wholistic nursing assessments and interventions

·   Wholistic health education, promotion and referrals

·   Healthcare, housing and social justice system advocacy, navigation and referrals

·   Clinical opportunities for student health service providers

How You Can Help:

·   Donate medical supplies

·   Volunteer as a healthcare or social services professional

·   Become a Faith Community Nurse with HC4HP

·   Ask about clinical opportunities for health and social service students
